Actually this provides the ability to send any type of "SMS" or "Text" to the iPod Touch via some simple software available commercially. (free)

the new iPhone 3Gs has voice recognition for commands. Impressive. This could be worked into the effort without effort.

Perhaps we can add an alert for Prescription Eyewear pick-ups also. I noticed a long wait in the eye-clinic today for these pick-ups.

Abstract

The purpose of iVetGuide is to prove the utility of iPhone technology within the VA clinical environment by utilizing the iPhone's unique motion/location measurement system. iVetGuide is a prototype application that provide Veterans with navigation assistance and other location/speed/direction related services to enhance the Veteran's clinical experiences.

During outpatient visits at the Medical Center Veterans are tasked to navigate to various departments for clinical procedures and various care-provider appointments. These appointments may require the Veteran to traverse the medical campus during their visit. These appointments may include visits to the radiology, rehab, prosthetics, pulmonary lab, ECG lab, pharmacy and other locations. Veterans often become lost or confused and are frequently late for appointments. These conditions frustrate Veterans, increase appointment wait times, provide negative impressions of VA health care services, and most importantly can cause delays in treatment for the Veteran.

iVetGuide intends to demonstrate how the aforementioned issues can be alleviated and prove the technology's value in the clinical setting by providing the Veteran with one-on-one automated guest services including:

  • Personal visit schedule navigation (Inside/Outside GPS, visual map, stay on the path guidance)
  • Voice rescue/assistance from the information desk

iVetGuide has the potential for additional functionality of:
  • Here & Now - automatic appointment notification and registration based on physical proximity
  • Show Me - a visual 'look through the camera' with overlaid information called "Augmented Reality"
  • Wait Time - real-time notification of patient wait times at the patient's appointment locations
  • Time 2 Go - advance warning of travel times to next clinical appointment based upon current location
  • Pace Me - adjustment of travel time estimations based upon the patients real-time measurements of gait and pace
  • Here 2 - social networking connecting Veterans in the clinic/campus automatically
  • Survey Me - assess patient education, preappoinment assessment, and clinical surveys
  • Pick Up - pharmacy/prescriptions pickup notifications
  • Teach Me - patient education via clinically distributed podcasts and vodcasts
  • Find Me - campus patient locator, two way location (patient finds location, Doctor finds patient)
  • Support Grouping - support group social networking

Technology Evaluation

Based on the redesign of the new iPhone 3Gs and new iPod Touch along with the 3.0 operating system for both, new opportunities and applicabilities to healthcare have become available which will enhance the patient and provider experiences. In regards to the abstract design the 3Gs has an internal guidance capbilities including WiFi triagulation, cellphone tower triagulation, GPS and directional compass that will be a good platform for the patient navigation tool. The iPhone 3Gs has a built-in voice command recognition features that complies with Section 508.

Impact

24 million enrolled Veterans and their caregivers that come to the medical center each day. It affects not only clinical providers but all the ancillary services provided to the Veteran as well. The goal of this device is to get the right patient, to the right location, at the right time.

Technical Approach

Build an iPhone application that utilizes the location services (GPS, others) along with individal cllnical environments visual and radio mapping to guide the Veteran through their patient-specific clinic visit agenda. The software is Veteran centric and can be downloaded by secure wireless to their personal devices or to devices provided at the medical center. In order for this function to be built, the system will require access to VistA in order to query patient to do listing (patient's orders for consults, procedures, pharmacy, radiology, and lab) for extraction and presentation to the iVetGuide iPhone application's web interface. This data will then be transmitted securely to a patient's iPhone or Touch without personal identifiable information.

As a use scenario, the Patient walks into hospital lobby with either their own device or one that is provided by hospital information desk (with easy instructions on how to use). The Patient logs into iVetGuide and is provided with their first clinical appointment such as an appointment with their primary care physician. This appointment goes into the built in calendar on the iPod/iPhone to leverage the alarming and alerting built into the system. At the proper time, the Patent is alerted to their upcoming appointment. The alert timing is adjusted to accommodate the physical distance the patient is from their appointment. If there is a backlog at the Patient's appointment or if it has been determined that the Patient needs more time to walk or travel to their appointment, the alerting time is also adjusted to accommodate. The Patient is then guided by iVetGuide to their initial appointment. If the patient is in route but potentially late based upon their rate of travel, the predicted delay is forwarded to the appointment (e.g. Doctor, lab tech, other) and the patient is notified that their designated contact has been informed of the delay. At the close of the Patient's initial clinic appointment, iVetGuide automatically receives an update via the clinician's input as a result of clinical appointment. The iVetGuide then guides the patient to the various locations as prescribed by the clinician. For example - the doctor orders the patient to obtain an EKG on the 4th floor, stop by the physical therapy department in the basement and pick up the prescriptions at pharmacy on the ground floor of the adjacent campus building. The patient will be guided to find these different locations in an order based upon the appointment times, the anticipated wait times at each location, the Patient's ability to reach each location (e.g. their locomotion speed and capability), the time those departments close, and otherwise. On completion of their tasks, they will return the device as they exit the hospital (it will remind them to do so / signal the hospital that a device is leaving the premises) or take personal device home and it will clear the information once it leaves the hospital boundaries based upon its location outside the VA campus/facility.
